MAIN MENU

In support of this we provide an Information, Advice & Guidance (IAG) Service to all clients, delivered by fully qualified and experienced staff. This service meets the national Matrix IAG Standards.
Birmingham Rathbone’s services are provided in partnership with Birmingham City Council’s Vulnerable Adults, Health & Social Care and PURE Teams, as well as the Care Quality Commission, Housing Associations and private landlords. We provide a variety of accommodation across the City and can also provide care or support to people living in their own homes or tenancies.
All those applying to access our services are invited to complete a referral form. Please double click on the referral form link below to access the form. If you need help in filling in the form you could ask a professional (e.g. social worker, care worker etc.) or a relative or friend to support you, or contact us to help you complete it.
Having a completed referral form enables us to undertake an accurate and comprehensive needs assessment to ensure we provide appropriate person-centred support/care to each person. Information, Advice & Guidance is delivered as part of this process and throughout the duration of the client’s support/care.
